Job Description: Duties and Responsibilities: Under the general supervision of the Skilled Nursing Facility Administrator: Ø Perform evaluations, screens and treatment for all residents with medical referrals. Ø Determines nature and extent of impairments. Ø Provide appropriate therapeutic treatment programs for all medically referred residents. Ø Maintains treatment records on all residents referred to physical therapy. Ø Attend staff and inter-disciplinary team meetings. Additional Information:
