Job Description:
Under the direct supervision of the RCFE Administrator, or designee: Make rounds as requested and immediately report unusual resident conditions to physician or RCFE Administrator and Clinic. Establish consistent routine. Transcribe/note physician’s orders. Administer medications and treatments as per physician’s orders. Evaluate medication administration schedule to assure proper absorption, monitor for side effects of prescribed medications, vital signs and resident behavior. Monitor for signs and symptoms of dehydration. All nursing care and treatment plans shall be recorded properly on the Medication Administration Record (MAR). Assume responsibility for completing consents, laboratory reports, and charts. Must adhere to the policies and procedures of the facility. Complete assigned weekly summaries, as scheduled Documentation in these summaries will reflect the resident care plan. Documentation will be done on each shift of resident care, as scheduled. Identify nursing problems including physical, emotional and social needs and begin formulating a written plan of care. Assist with initial resident interview and assessment upon admission, discharge, transfer and death of a resident. Assist professional technicians before, during and after residents treatment program. Identify rehabilitative potential of individual residents; plan care directed toward attaining optimum level of function. Assure that appropriate vital signs are taken and recorded as indicated for level of care assigned. Assess for signs and symptoms of pain or discomfort, use nursing measures of comfort and medicate resident as needed. Participate in nursing care to meet emergency situations, assist in providing a safe environment for the residents. Provides specific instruction and guidance with regard to medication management. Participate, as necessary; in daily resident care conferences on unit, planning for and evaluating care for residents. Participate in change of shift report and communicate pertinent information to the team. Function as a team leader over-seeing the work of the Certified Nursing Assistants in executing the plan of care for the residents. Motivate and assist residents to develop self-reliance in activities of daily living. Allow rest periods to minimize fatigue. Answer call-lights promptly. Check controlled drugs inventory. Responsible for checking emergency drug supplies. Maintain medication refrigerator, medication room and medication and treatment carts, clean and in order. Is beneficial having knowledgeable of 22CCR, Division 6, Chapter 8 (RCFE) requirements and provides on-the-job training to RCFE staff to ensure compliance with these regulations. Training areas include, but are not limited to: The aging process and physical and special needs of the elderly, Importance and techniques of personal care services, Residents’ rights, Policies and procedures regarding medications, Psychosocial needs of the elderly, and Recognizing signs and symptoms of dementia in individuals. Documents staff training. Performs skills checks for C.N.A. staff. Assists with reviewing and evaluating systems in place within the RCFE and advises the RCFE Administrator, or designee on recommendations for systems improvements. Participates in the implementation of systems improvements and ensures staff is instructed on and competent in new processes and procedures. Assure that all residents have monthly/weekly weight taken and recorded; report significant changes to the physician and registered dietitian immediately. Assists in the review of RCFE policies and procedures. Provides input to the supervisor. Ensures staff receives instruction on and is competent in implementing any new processes and procedures that result from new or revised RCFE policies or facility policies that impact the RCFE. Reviews and orders training materials to include books, video instruction tapes, interactive CD-ROM’s and similar materials. Develops handouts and written training materials. Support the effort of other therapies by effectively carrying out restorative nursing measures established. Teach basic health measures, activities of daily living and carry out other routine aspects of care. Ensure maintenance or restoration of correct body alignment and mobility, resident’s personal hygiene and skin integrity. Ensure that staff utilizes optimum standards of resident care and that staff and members observe safety and Infection Control Precautions and regulations at all times and stimulate a positive attitude toward these standards. Participate in nursing care to meet emergency situations, assist in providing a safe environment for the residents. Be aware of resident’s rights, identifying the potential for resident abuse, report promptly any adverse behavior, resident abuse or suspected abuse by staff. Participate in continuing education programs to maintain and upgrade nursing knowledge and skills; make suggestions through appropriate channels for revising procedures and policies when indicated. Other LVN duties as assigned to meet resident and facility’s needs.
