Job Description: Pending Freeze Exemption Approval Multiple positions MAJOR TASKS, DUTIES, AND RESPONSIBILITIES Works as a team member with other treatment disciplines; provides recovery oriented courses, therapeutic interventions, and other social services; assists the treatment team in evaluating individuals’ readiness for release; works as liaison between Conditional Release, family, community and the team. Direct Treatment Conduct or co-lead recovery-oriented courses/groups whose focus is dictated by individual need as defined in the treatment plan and provided according to therapist skills and approved hospital design. This includes being Primary Provider of co-provider of ten hours of recovery-oriented treatment courses/groups per week. Meet with individuals for regular therapy in which the focus is dictated by individual need as defined in the treatment plan and provided according to therapist skills. Maintain a therapeutic relationship with individuals. Provide on-going information regarding treatment expectations, legal issues and progress towards the next dispositional setting. Meet with and provide social work services for individuals who have identified needs. Develop and maintain contact with individuals’ families as appropriate and provide them with information when the individual allowing such contact has signed the proper release forms. Provide crisis intervention services. Participate in community meetings of all unit members, as requested. Provide collaborative services with relatives and community agencies, including face-to-face, letter and phone contacts. Assessment, Planning, and Evaluation Participate in Treatment Team meetings as required for evaluation and development of each individual’s Wellness and Recovery Plan for an assigned caseload. Develop, review and update the social work component of the integrated assessment with each new individual, consistent with hospital policy and standards. Provide input to the Treatment Team regarding new and pertinent information that may have an impact on the individual’s treatment course. Assist in identifying treatment needs, designing and developing individualized treatment plans, assessing individual progress and making recommendations for discharge placement as appropriate. Complete all course/group evaluations, participation ratings, and other standardized measures, as required. Participate in all 7-day, 14-day, and 30-day reviews of each individual on your caseload or assigned service area, provide input on progress in treatment; report on family, social and community resources. Review chart prior to each 7-, 14- and 30-day Treatment Planning Conference (TPC) for individuals on assigned caseload. Help identify further treatment objectives; and make recommendations for treatment priorities. Prepare and update treatment plans that reflect individuals’ current and ongoing treatment needs, including dictation of the narrative summaries for individuals on assigned caseload. Review CONREP reports, independent evaluations, Court documents and update the Treatment Team as needed. Participate in mini-team meetings, and shift change meetings, to exchange progress information and participate in relevant in-service training provided during these meetings. Write monthly social work notes describing and evaluating the individuals’ responses to social work services received as well as primary clinician and treatment services responses according to MHDS requirements. Notes are to include description of direct treatment, other contacts with individual and family, agency contacts, and phone calls regarding the individual. Insure initial safety risk assessments are completed and update assessments as required by policy. Program Evaluation and Development Participate in Program social worker meetings as scheduled. Participate in Treatment Design Team meetings and serve on work groups as requested by management. Attend and participate in staff meetings, staff development sessions, placement meetings and, pursuant to caseload and agenda, the specific Forensic meeting. Serve as case manager by coordinating treatment for specific individuals as determined through the Treatment Team. As case manager, meet with the individual’s treatment providers as applicable. Be available to management for Social Work consultation on specific questions of individuals’ progress and contribute to assessments of population characteristics, treatment needs, treatment resources, and staff development needs. Discharge Planning Collaborate with the Treatment Team, State’s Conditional Release Program, Department of Corrections and/or Parole Agents in formulating a community treatment contract/discharge plan and facilitating an individual’s discharge to his dispositional placement setting. As applicable, facilitate application for Supplemental Security Income, social security benefits, California ID card, or driver’s license prior to discharge. Assist in assuring submission of conservatorship applications, LPS holds and certifications. Complete affiliated paperwork for discharge planning, i.e., Social Services Discharge Summary or Recommended Continuing Care Plan. Deliver the completed summary to the Health Information Management Department. Provide emergency social work services as needed to other units or service areas. Professional, Resource, and Training Functions Regularly attend meetings of the hospital Social Work Department. Keep current on social, legal, and clinical literature and resources through workshops, classes, and other means of study. Maintain a current license as a clinical social worker. Act as a resource to individuals and staff on community resources, e.g. social, educational, vocational, financial, housing, and the like. Assume advocacy role with individuals in apprising them of their treatment and available resources. If qualified and with approval form the Social Work Service and management, supervise social work associates, interns and social workers preparing for licensure as part of a professional’s approved clinical-supervision plan. Provide orientation and ongoing assistance to new Social Workers, as requested. Additional Information:
