Located in the heart of scenic Napa Valley, the Veterans Home of California, Yountville is a community of and for veterans. Some 1,100 veterans (both men and women) live at the home. Founded in 1884, the Veterans Home of California, Yountville is the largest veterans’ home in the United States. It provides residential accommodations and a wealth of recreational, social, and therapeutic activities for independent living. Location: Veterans Home of California – Yountville 110 California Drive, Yountville, CA 94599 Licensed Vocational Nurse (8249) Full – Time, Part-Time, Intermittent; Permanent – Continuous Filing $3333.00-$4051.00 (Includes a compensation benefits package) NOTE: Hiring for this vacancy is contingent upon obtaining all approvals prior to making a commitment Description: Under Supervision, to perform the following; the predominant duties for this position include, but are not limited to: *Makes rounds with physician as requested and reports unusual patient conditions to physician or nursing supervisor without delay. Transcribes doctor’s orders and makes out lab and x-ray slips. Gives medications and records in proper areas on Nurses’ Notes. Assumes responsibility for seeing that consents, laboratory reports and charts are complete for patient going to surgery or for special treatments and that patient’s are properly prepared. *Assist with initial patient interview and assessment on admission and explains role of various staff members on the unit in absence of RN; does nursing history and assessment, orients to physical layout of ward, specific ward routine and use of call bell, assures that physical inventory of patients belongings is completed. *Assist in formulation and revision of nursing care plans. Identifies rehabilitative potential of individual patients and plans care directed toward attaining optimum level of function as demonstrated by maintaining proper body positions and body alignment of bedfast patients, maintaining adequate nutritional fluid intake, maintaining bowel and bladder training program, assisting patients in adjusting to their disabilities and redirecting their interests, assisting patients in the techniques of ambulation and carrying out exercise as directed by the physical therapist between patients’ visits, assisting and teaching the activities of daily living (feeding, dressing, grooming and toilet activities, assisting in teaching remediation and reality orientation where appropriate. *Completes assigned weekly summaries as scheduled by the SRN on SNF and ICF wards. Documentation in these summaries will reflect the patient care plan. Assure that appropriate vital signs are taken and recorded as indicated for level of care assigned. Assures that all patients have monthly weight taken and recorded and reports any significant change to the physician immediately. *Participates in multidisciplinary team meetings as indicated on a weekly basis and ensures updating and review of every patient care plan on a quarterly basis on SNF and ICF wards. Participates and may conduct as necessary daily patient care conference on acute ward. Participates in change of shift report and communicates pertinent information to all members of nursing team. *Familiarizes staff with optimum standards of patient care and stimulates a positive attitude toward these standards. Insists that safety precautions and regulations for patients and staff be observed at all times. *May give direct personal patient care as assigned by the RN. *Working Overtime. *Other duties as related.
